If you are not familiar with the tools and resources below, please take a few minutes to review. These services are here to help get your Plan Members their information and claims payments faster and in the way they want.

Plan Members choose how they want to submit claim forms:

Plan Members can fax claim forms and information to us, take a pen to paper and mail them to us or scan forms and email them to us, the choice is theirs. Our health and dental claim forms are available on your Innovations web services site at EquitableHealth.ca or on our public web site at Equitable.ca.  These self-contained forms include all the information that Plan Members need to get your claim form to us.

Plan Members choose thier language (French or English):

Whether Plan Members call us with a question or go online, our services, forms, information and staff are always ready to serve in French or English.

Plan Members choose how you want to contact us:

Plan Members can call us, email or take a pen to paper with any question they may have about thier benefits and our staff will be glad to help out. A complete list of our departments, hours of operation and contact information is available at equitable.ca.
