How to Submit a Health or Dental Claim to Equitable Life
How do you submit a Health or Dental claim to Equitable Life?
Well, that depends on what works best for you. You can submit your
Health and Dental claim forms and associated receipts through
Equitable EZClaimTM (Mobile or Online) or by
fax or regular mail.
Please note that whichever way you choose to submit your Health
or Dental claim, you always need to submit your receipts at the
same time. If we do not receive your completed claim form and
receipts at the same time, we will not be able to process your
claim.
Equitable EZClaim Mobile - Wireless Device
Claims
If you have a wireless device (AndroidTM, iPhone, iPad2 or
BlackBerry®), you can submit Health and Dental claims
and receipts using Equitable EZClaim Mobile.

Equitable EZClaim Online -
Paperless Claims
Paperless claims allows you to use a savable PDF to complete
your claim form and save it directly to your computer. This way,
you can either use Equitable EZClaim Online or email
to submit your Health and Dental claim and associated receipts
directly to us in an electronic format.

site or your secure Plan Member Web Services Site
(EquitableHealth.ca)*.

(name, address, Policy Number and other required information) will

Health and Dental Claims - Fax or Regular Mail
If you are not inclined to use Equitable EZClaim, you
can always fax or mail your Health or Dental claim to us. All you
have to do is open and complete the appropriate claim form above
(#466 or #520) and fax or mail it to us. You will need to sign the
form before you fax or mail it to us along with the related
receipts.
